The following health plans are proud supporters of SilverSneakers and offer the SilverSneakers® Steps program as a benefit to select members of their health plan.

Please contact your health plan at the number below to determine if you are eligible for SilverSneakers® Steps.


SilverSneakers® Steps is available to you if you are an Arizona State Retirement Plan Medicare Eligible member enrolled in PacifiCare Senior Supplement or an Early Retiree enrolled in PacifiCare PPO and residing outside of Maricopa County, Pima County, Flagstaff County, Prescott County, Casa Grande or Yuma County.

Contact PacifiCare Senior Supplement member services at 1-800-851-3802, TDH 1-800-647-6038, 7AM to 9PM Monday through Friday, to determine if you are eligible.
 

SilverSneakers® Steps is available to you if you are a member of Secure Horizons Medicare POS Plan in Arizona or Nevada. For more information, contact Secure Horizons Medicare POS Customer Service at 1-866-302-1979, (TTY) 1-800-647-6038 Monday through Friday, 7:00 a.m. to 9:00 p.m.
 

Eligible SecurityBlue, FreedomBlue, and Medigap Blue members of Highmark Blue Cross Blue Shield living 15 miles or farther from the closest fitness center can participate in the SilverSneakers® Steps program. To determine if you are eligible, please call:

Eligible Medigap Blue members of Highmark Blue Shield living 15 miles or farther from the closest fitness center can participate in the SilverSneakers® Steps program. To determine if you are eligible, please call:

Eligible members of Humana Gold Choice Private Fee For Service (PFFS) Plans living 15 miles or farther from the closest fitness center can participate in the SilverSneakers® Steps program. To determine if you are eligible, please call Humana at 1-800-457-4708, 1-800-833-3301 TDD.
 

 

SilverSneakers ® Steps is available to you if you are a Seniors First Medicare Advantage member in Alabama residing 15 or more miles from a participating SilverSneakers fitness center. For more information, please contact HealthSpring of Alabama at (800) 952-1992, TTY (877) 262-9090 (You need special telephone equipment to use this number.), M-F 8 a.m. to 5 p.m.

 

SilverSneakers ® Steps is available to you if you are a HealthSpring Medicare Advantage member in Tennessee residing 15 or miles from a participating SilverSneakers fitness center. For more information, please contact HealthSpring at (888) 501-3965, T TY: 800-391-9806, M-F 8 a.m. to 5 p.m. CST
and 9 a.m. to 6 p.m. EST.

 

SilverSneakers ® Steps is available to you if you are a Texas HealthSpring Medicare Advantage member residing 15 or more miles from a participating SilverSneakers fitness center. Empire’s Senior Plan Direct, Senior Plan Direct PPO and Direct Payment Medicare Supplement members who reside 15 or more miles from the closest fitness center can participate in the SilverSneakers Steps program. To determine if you are eligible please call:
